UnitedHealthcare Community Plan of Arizona
UnitedHealthcare Community Plan is one of the two DDD Health Plans, providing physical health, behavioral health and Children's Rehabilitative Services to Division of Developmental Disabilities members on ALTCS across Arizona. It separately holds an ALTCS long term care contract for people who are elderly or have a physical disability in eight counties, and an AHCCCS Complete Care contract in the Phoenix and Tucson areas. For DDD members it covers acute care while DDD's Qualified Vendors deliver the long term services and supports.
