Yolo Community Care Continuum works to better the lives of people with mental illness through direct services, advocacy, education, and volunteer efforts, with client driven services in Davis and Yolo County. It operates the Cornerstone and Safe Harbor short term crisis residential facilities, the Farmhouse adult rehabilitative residential facility, Harmony House adult residential facility, the Haven House transitional care program, and permanent supported housing.
