Aging and Disability Resource Connection (California ADRC network)
The Aging and Disability Resource Connection is California's 'no wrong door' system for long-term services and supports. Local ADRCs are partnerships, usually between an Area Agency on Aging and an Independent Living Center, that provide enhanced information and referral with warm hand-offs, person-centered options counseling, short-term service coordination for people at risk of institutionalization, and transition services for people moving out of a hospital or nursing facility back into the community. ADRC services are open to anyone regardless of age, income or disability.
