Las Animas County Department of Human Services, Case Management Agency
A designated Case Management Agency under Colorado's Case Management Redesign. It is the single point of contact in its defined service area for case management across all Home and Community Based Services waivers: functional eligibility assessment, person-centered service planning, authorising waiver services, monitoring and annual reassessment. Under conflict-free case management rules it does not provide the direct services it authorises.
