ElderSource, The Area Agency on Aging of Northeast Florida
This is the Area Agency on Aging and Aging and Disability Resource Center for Planning and Service Area 4, covering Baker, Clay, Duval, Flagler, Nassau, St. Johns, Volusia. Florida's 11 AAAs are also its ADRCs, so this one organization is both the Older Americans Act planning and funding body for its area and the single front door for information and access to long-term care services. It provides information and referral, screens and determines eligibility, triages people needing help, and manages the funds for key long-term care programs targeted at elders. It contracts with local providers rather than delivering most services itself, funding home-delivered and congregate meals, adult day care, personal care, respite, transportation, caregiver support and legal assistance.
