Hawaii Developmental Disabilities Division - Oahu Intake and Case Management Sections
This is the island section of the Hawaii Department of Health Developmental Disabilities Division. It takes applications for DDD eligibility, carries out intake, and provides the case management that opens access to the Medicaid I/DD Waiver and to state-funded DDD programs for residents of this area. Case managers write the individualized service plan and authorise the individual support budget.
