Money Follows the Person (Iowa Partnership for Community Integration)
Money Follows the Person is a $51 million CMS-funded program that helps Iowans move out of institutions and into homes in the community of their choice. It is open to people with an intellectual disability or brain injury who have lived at least 60 consecutive days in an Intermediate Care Facility for Individuals with Intellectual Disabilities, a nursing facility, a Psychiatric Medical Institution for Children or an inpatient hospital setting, and who want to move. Grant funds pay for transition services and enhanced supports during the first year after the move. The University of Iowa Center for Disabilities and Development works on the program alongside Iowa HHS.
