Western Iowa Service Collaborative (Disability Access Point, Districts 1 and 4)
The Western Iowa Service Collaborative is the contracted Disability Access Point for HHS Districts 1 and 4, covering 27 counties in western and southwest Iowa. It is administered by Pottawattamie County. As a Disability Access Point it provides information and assistance, options counseling, service coordination, and short-term and long-term services and supports for people with disabilities, their families and caregivers, and is part of the Aging and Disability Resource Center network.
