Indiana Bureau of Disabilities Services - District 2 Field Services Office (South Bend)
District 2 is the Bureau of Disabilities Services field office for 11 Indiana counties. District office staff take applications for BDS services, determine whether a person meets the developmental disability definition and the ICF/IID level of care, hold the intake meeting when a Family Supports Waiver slot is offered, give families the provider choice list, and are the local contact point for Community Integration and Habilitation Waiver priority requests and Supervised Group Living placements.
