Area IV Agency on Aging & Community Action Programs, Inc. is the Area Agency on Aging and Aging and Disability Resource Center for Indiana's Planning and Service Area 4, covering 8 counties. As an ADRC it is the entry point for the Health and Wellness and PathWays Medicaid waivers: it completes the nursing facility level of care assessment, places people on the waiver waiting lists, helps with the Medicaid application at no cost, and provides options counseling, care management, home-delivered meals, caregiver support and transportation. It is part of the INconnect Alliance, Indiana's statewide ADRC network.
