CIRCARE
Onondaga Case Management Services, known as Circare, provides care management and community based services in the Syracuse area for people who face barriers to their mental and physical health. Adult services include care management, rehabilitation and recovery services, vocational services, peer services and assertive community treatment, and its children and family services include health home care management, home based crisis intervention and youth assertive community treatment.
