Baylor Transition Medicine Clinic
The Transition Medicine Clinic at Baylor College of Medicine provides healthcare and social support services to young adults and adults with developmental disabilities, many of whom previously received care at Texas Children's Hospital. The team helps patients move into adult primary care, navigate healthcare systems and identify the social services they need. The clinic also runs a peer mentorship program for people with intellectual and developmental disabilities.
